I understand that you will exercise precautions to assure the animalʼs safety while it is in your care, but will not hold you Kellyʼs Kennels L.L.C. or employees responsible if my pet should injure itself, escape, fail to eat, become ill, or die. I also authorize the following or attached emergency plan where emergency circumstances where time is critical to follow through with such procedures as are necessary for the well-being of my pet.

Boarding Form

All Animals must have verification of vaccination within the past year and be free of internal and external parasites (Worms, Fleas, Ticks). The animal must be on a veterinarian-approved flea and tick prevention program. There will be an additional charge if we apply the flea and tick prevention product.

We recommend that the animal be kept on its familiar diet. Boarding and sudden change in diet can cause stress. The cause of bloat is unknown and life-threatening. Please provide a written emergency plan that is to be followed should your animal become ill and need veterinarian care during their stay with us.
